Initial visit - in-person or by telemedicine
You can be prepared by downloading the packet of forms that you are likely to need, printing the pages, and filling them out before you have your first appointment. Apointments are available either in-person or by telemedicine. You are welcome to receive these forms at our office on your first visit if in-person, or online here if your first visit is through telemedicne. |
